We are looking for an experienced, committed and motivated professional to work with older people who are living with dementia or functional illnesses.
We are a friendly and supportive team and encourage professional and personal development
Main duties of the job- Complete Memory Assessments (training will be given)
- Complete initial assessments
- Participate in Duty Rota
- Nursing Needs Assessments

- 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years
- Experience of working with older adults with mental health needs
- Experience of working in a Community setting
- Experience of working with service users with organic and functional illnesses
- Experience of supervising junior staff/students
- Understanding of the role of Community Mental Health Teams.
- Up-to-date knowledge and understanding of relevant mental health legislation and issues.
- Registered to practice/evidence of revalidation
