About the Position- Full-Time role with flexible hours
- £60,000 per annum - D.O.E.
- Provide medicines management and structured medication reviews
- Participate in service improvement and multidisciplinary working

- Independent Prescriber needed
- Must be registered with GPhC
- At least 2 years' UK experience in community
- CPPE qualification is a bonus
- Confident managing vulnerable patient groups
- Essential knowledge of common acute and long-term conditions
- Comfortable in a busy, outcomes-driven environment
- Excellent clinical communication and collaboration skills
