Practice Nurse

As a Practice Nurse, you will:

Deliver routine nursing care including immunisations, cervical screening, wound care, and health checks.

Contribute to the ongoing management of patients with both acute and long-term conditions.

Provide health promotion and lifestyle advice.

Maintain accurate clinical records in line with practice protocols.

Work collaboratively with GPs, healthcare assistants, and administrative staff.

• Undertake comprehensive health assessments, including physical examinations and health screening.
• Deliver evidence-based health promotion and lifestyle advice to patients.
• Administer vaccinations and immunisations in line with national guidelines.
• Support and manage patients with long-term conditions such as diabetes, asthma, and COPD.
• Contribute to chronic disease management clinics and clinical audits.
• Work collaboratively with GPs and the wider clinical team to ensure high standards of care.

• Minimum of 2 years experience working as a Practice Nurse in general practice.

• Registered Nurse with a valid NMC PIN.
• Non‑medical prescriber V300, or willingness to work towards.
• Proven experience in chronic disease management.
• Excellent communication and interpersonal skills.
• Ability to work autonomously as well as part of a multidisciplinary team.
