Overview
We are seeking a resilient, flexible, and adaptable Practice Nurse to join our clinical team. This position is ideally suited for a qualified nurse with a minimum of two years post-registration experience. Whether you have an established background in primary care, community nursing, or an urgent treatment centre, or you are currently managing long-term conditions in secondary care and looking to transition into General Practice, we want to hear from you.
You will be a confident practitioner, comfortable working autonomously without direct supervision, and capable of making and reporting independent clinical decisions. Ideal candidates will hold additional qualifications or equivalent experience in a Diploma in the management of high-demand clinical specialties relevant to our patient demographic (e.g., Diabetes, Cardiovascular, Respiratory).
- Provide routine nursing care in accordance with clinical evidence, local, and national guidance.
- Implement and evaluate individual treatment plans for chronic disease patients, maintaining accurate chronic disease registers.
- Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health effects.
- Run chronic disease clinics, referring patients to secondary or specialist care when required.
- Support patients with the safe use of prescribed or over-the-counter medicines within your scope of practice.
- Provide comprehensive travel medicine services.
- Request and process pathology services as necessary.
- Prioritise health issues and intervene appropriately, supporting the team during clinical emergencies.
- Recognise, assess, and refer patients presenting with mental health needs.
- Develop, implement, and embed health promotion and well-being programmes, delivering opportunistic health promotion where appropriate.
- Delegate clinical responsibilities safely and appropriately within the scope of practice of individuals.
- Liaise with external agencies to ensure vulnerable patients are fully supported.
- Support the clinical team with all safeguarding matters in accordance with local and national policies.
- Maintain exceptional and accurate clinical records.
