We are looking for someone who can help clients live independently, maintain their homes, and engage in meaningful activities. You’ll support clients to build skills that enable them to access work, training or education and community‑based activities, as well as promoting financial inclusion, healthy lifestyles and meaningful engagement.
In This Role, You Will be Responsible For- Using techniques such as coaching and strengths‑based approaches to enable clients to develop emotional resilience and overcome barriers.
- Leading liaison with external agencies and raising any safeguarding issues appropriately and in a timely manner.
- Completing the Positive Behaviour plan, Support plan, Essential profile and Risk assessment for customers.
- Managing workload and client numbers based on client needs and service demand.
- Supporting the Project Manager in ensuring that all aspects of housing management, housing‑related support and enablement support are carried out to the highest standard.
- Working as part of a team within supported housing and outreach services, providing high‑quality, personalised housing‑related support to young adult clients on an outreach basis with a diagnosis of ADHD and ASD, and outreach clients with substance misuse issues.
- Knowledge and understanding of the needs of complex‑needs and vulnerable adults.
- Knowledge of the issues that can impact on the lives of clients: behaviour management, understanding diagnoses, leaving care and mental health.
- Experience working in a supported housing environment or relevant social care setting.
- Experience working with young people and vulnerable adults with complex support needs.
- Experience working with statutory agencies and voluntary support services.
- Ability to drive and have access to a vehicle.
