Role Overview

We are seeking a Discharge Support Worker to join our Mental Health Floating Support team, working across wards and local community outreach sites in North and South Manchester.

This role delivers a highly responsive hospital in‑reach service to provide housing‑related support for people with mental health needs who face barriers to discharge such as homelessness, a history of offences or financial difficulties. The successful candidate will support individuals to achieve outcomes in independent living, social skills, community links, social inclusion and pathways to meaningful employment.

Key Responsibilities
  • Carrying out person‑centred assessments to identify housing support needs
  • Ensuring clear targets for outputs, timescales and quality
  • Providing direct support and practical assistance in accessing housing and support options
  • Providing support to maximise financial wellness
  • Creating links with housing and support providers to establish sustainable pathways
  • Ensuring smooth transitions out of hospital
Requirements

Due to the nature of this role, experience in supporting individuals with mental health needs is an essential requirement.
