Supply Care Solutions is recruiting an experienced Family Hub Worker to join Trafford Council's Early Family Help Service. This is an exciting opportunity to be part of a newly developed Family Hub model focused on delivering the right support at the right time for children, young people, and families.

Working within the Directorate for Education, Standards, Quality and Performance, you will support families through early intervention, helping improve outcomes and ensuring children can grow up in safe, supportive environments where they can thrive.

Key Responsibilities

* Provide early family help support to children, young people, and their families through Trafford's integrated Family Hub model.

* Work in partnership with professionals including Health Visitors, Schools, Early Years Providers, Housing Services, Police or Other key agencies

* Act as a Key Worker, managing cases and coordinating tailored support plans.

* Complete whole-family assessments and interventions using agreed systems and procedures.

* Support children and families with emerging developmental differences and SEND needs.

* Deliver and co-facilitate group and individual programmes for families.

* Plan activities that support children's development, learning, and wellbeing.

* Maintain accurate records and case notes in line with departmental policies.

* Monitor and evaluate intervention outcomes to ensure positive change for families.

Family Hub Worker Requirements

 1. A recognised Level 3 qualification in a relevant field or Significant experience working with Children/Young People, Family Services, Early Years, SEND provision, Educational setting

 2. Current Enhanced DBS Disclosure

 3. Eligibility to work in the UK without work permit or visa restrictions

 4. Strong communication skills, excellent organisational abilities, and a passion for improving outcomes for children and families
