Are you passionate about supporting people to live independently and improve their health and well-being?

An exciting opportunity has become available for 2 Temporary Living Well Coordinators to join a supportive community-focused team. This is a fantastic opportunity for someone with experience supporting vulnerable people who enjoys building positive relationships and helping customers achieve greater independence.

As a Living Well Coordinator, you'll provide person-centred support to customers, carrying out well-being visits, helping people access appropriate services and working alongside colleagues and external partners to ensure customers receive the right support.

What will you be doing as a Living Well Coordinator?

  • Visiting customers to check on their well-being and provide appropriate support
  • Developing individual support plans based on customer's needs
  • Supporting customers to maintain their independence and access appropriate services
  • Responding appropriately to well-being and safeguarding concerns
  • Working with colleagues, support providers and external agencies to coordinate effective support
  • Supporting customers with day-to-day issues and helping them access the right teams and services
  • Supporting customers with physical and mental well-being and helping to reduce social isolation
  • Working with customers across sheltered housing, extra care and learning disability schemes
  • Carrying out regular checks and reporting any health, safety or maintenance concerns
  • Supporting events and activities that encourage customer involvement and community engagement
  • Maintaining accurate case notes and records
  • Using your initiative to resolve issues and escalating concerns where necessary
  • Managing your own workload and working flexibly across different schemes

We would LOVE to hear from you if you have the following skills and experience:

  • Previous experience as a Living Well Coordinator, Independent Living Coordinator, Support Worker, Housing Support Worker, Community Support Worker, Well-being Coordinator, Housing Officer or similar
  • Experience supporting vulnerable, elderly or disadvantaged people
  • Excellent communication and relationship-building skills
  • An empathetic, patient and person-centred approach
  • Understanding of safeguarding and the ability to respond appropriately to concerns
  • Experience working with multiple agencies or support services would be advantageous
  • Strong problem-solving skills and the confidence to use your own initiative
  • Excellent organisational skills and the ability to manage your own workload
  • Comfortable working independently as well as part of a wider team
  • Good IT skills and confidence learning new systems
  • A practical and resourceful approach to resolving day-to-day issues
  • Understanding of confidentiality and GDPR
  • Full driving licence would be advantageous
  • Similar roles could include:
