What you will do

• Provide essential statutory social work services to vulnerable adults and their carers, following local and national policy guidelines.
• Collaborate in multi-disciplinary team settings to deliver integrated care solutions effectively.
• Safeguard adults through managing risks, documenting, and sharing vital information with partners.
• Develop and execute person-centred care and support plans, using a strengths-based approach to meet individual needs.
• Contribute to the development of the team by participating in activities and providing support under the direction of the Team Manager or Advanced Practitioner.

As a member of Salford City Council’s Adult Social Care and Health Partnerships, you will be at the forefront of delivering critical social work services in mental health.
