The Mio Care Group in Oldham is recruiting a Transfer of Care Officer (Community Reablement) to join the Transfer of Care Hub based at Royal Oldham Hospital. You will help support referrals and triage following discharge from acute settings.
You will manage a designated caseload, lead on Discharge to Assess services, and assist with community enablement and bed-based placements to meet patient needs. The role requires collaboration with acute, community and partner organisations seven days a week
