Adults Social Worker- Hospital Discharge

Our client, a Great Manchester Council, is seeking an Adults Social Worker to join their Hospital Discharge Team. Fantastic payrate of £30 per hour! Complete Care Act assessments and support planning for individuals preparing to leave hospital. Work closely with health professionals, discharge coordinators, and community services to facilitate smooth transitions. Identify risks and safeguarding concerns, implementing robust plans to protect vulnerable adults. Arrange care packages, placements, and community support to promote independence and well-being. Ensure compliance with the Mental Capacity Act and best interest decision-making where required.

Maintain accurate records and contribute to multi-disciplinary discharge planning meetings. Respond promptly to urgent cases and manage competing priorities in a fast-paced environment. Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England. Must have minimum of 3 years post qualifying social work experience
