Overview
ACT Nurse (LPN) – As a member of the Assertive Community Treatment (ACT) Team, you will provide care to adults with severe mental illness in homes, workplaces, and communities. You will work with a multidisciplinary team to help clients live productive lives while staying integrated with family and community.
You will meet with clients in the home, community, and office to provide medical treatment and support services such as basic vitals and labs, facilitating administration of long-acting injectables, medication management support, and coordinating medical provider care. This role includes client transportation using a company car, a one week on-call rotation every 7-10 weeks (on-call can be virtual or in person, depending on the situation) and earns additional PTO.
You will primarily meet clients in the local community, with some time in the office or via Zoom/phone.
- $48,000-$51,000/year
- 8am-5pm weekday schedule, hours can be flexible

Assertive Community Treatment (ACT) was created in the 1970s and is one of the oldest and most widely researched evidence-based practices for adults with severe mental illness. By providing person-centered assistance including housing, recovery, vocational support, nurse/psychiatrist and therapist services, we enable clients to transition to lower levels of care or move toward independence, increasing quality of life.
What we’re looking for- Licensed Practical Nurse (LPN) licensed in Louisiana
- Experience providing treatment to adults with severe and persistent mental illness and/or substance use disorder
- Psychiatric experience preferred
- Valid driver’s license in good standing and current auto insurance
