Travel Physical Therapist Skilled Nursing Facility

Physical Therapist needed for a travel assignment in a skilled nursing facility located in Manheim, Pennsylvania, United States. The start date is October 12, 2026, for a 13-week assignment. Expect a guaranteed 35 hours per week with a pay rate of $2,001 to $2,103 per week. The shift is 5 days per week, 8-hour days (08:00 to 16:30). This position is travel - temporary in a skilled nursing setting.

• Conduct initial and ongoing physical therapy evaluations for residents
• Develop and implement individualized treatment plans to improve mobility, balance, strength, and functional independence
• Administer PT interventions including therapeutic exercises, gait training, and manual therapy as appropriate
• Monitor resident progress, document outcomes, and adjust care plans accordingly
• Collaborate with an interdisciplinary team, participate in care planning, and assist with discharge planning
• Ensure patient safety and maintain accurate records in accordance with facility policies

• Active physical therapy license to practice in Pennsylvania (or eligible)
• Experience or familiarity with geriatric or skilled nursing facility populations preferred
• Strong assessment, documentation, and communication skills
• Ability to work independently within a team-focused environment and adhere to the assigned shift
• Ability to meet credentialing and compliance requirements for a travel assignment
