1. This position is for Outpatient in a large clinic setting with a team of PTs, PTAs, CHTs and COTAs; general outpatient practice with the option to specialize if you would like.

Job SummaryEvaluates and treats all referred Physical Therapy patients according to physician orders. Home Care:
Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients. This position offers paid continuing education.
Degree in Physical Therapy required.
LicensureWisconsin licensed Physical Therapist or eligible required. If eligible, must obtain Wisconsin license within 3 months of hire. CPR certification is required.
ExperienceAt least one year experience in a medical setting preferred.
Special SkillsGood communication skills required.
