Superior Hospice is seeking a Social Worker to provide emotional support and advocacy to patients and families across the Twin Cities. You’ll serve as a steady, compassionate guide through end-of-life care, working with an interdisciplinary team that values purpose, presence, and partnership.

• Complete psychosocial assessments and develop care plans
• Provide grief counseling, therapeutic support, and crisis intervention
• Help families navigate end-of-life decision-making and community resources
• Collaborate closely with RNs, chaplains, and physicians
• Document visits and interactions with accuracy and empathy

• Hold a BSW or MSW from an accredited program
• Have previous hospice or palliative care experience
• Are an excellent communicator with a calming, supportive presence
• Know how to navigate both emotional complexity and regulatory compliance
• Have a passion for service and a deep respect for each unique end-of-life journey
