Nurse Practitioner/Physician Assistant Primary Care

Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.

Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.

• Review and sign MARs as requested by facilities, sign all primary‑care orders (including phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders) within 24 hours if faxed or emailed; review and address labs within 24 hours; manage narcotic prescriptions as per Eventus Whole Health Narcotic Rx Policy; provide adequate time each visit for paperwork and administrative duties.
• Provide on‑call triage duties per the on‑call triage protocol; adhere to updated triage schedules agreed upon by the Eventus Whole Health providers.
• Complete documentation and submit billing records per published charting and billing policies; provide the facility with a hard copy of the electronic documentation within 48 hours of service.
• Establish and maintain open and positive communication with facility staff and administration; provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information; meet with Administrator or designated facility care coordinator prior to the start of and prior to leaving the facility each day.
• Be knowledgeable of and adhere to Eventus Whole Health standards, policies, procedures, and all legal and regulatory agencies’ rules and guidelines, as well as professional ethical standards.
• Maintain patient confidentiality at all times, including appropriate use of cellphone, emails, text messaging, patient charts, and EMR; comply with HIPAA rules and regulations.
• Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.

• Masters degree in Nursing from an accredited institution and a current and valid license to practice medicine issued by the state medical board; all other necessary certifications must be current and valid.

• Have graduated from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs.
• Passed the national certification examination of the National Commission on Accreditation.
• Licensed by the state as a Physician Assistant.
• Specialization in geriatric, family, or psychiatry desired.
