ABL Health and Nottinghamshire County Council are partnering to deliver an Integrated Wellbeing Service across Nottinghamshire called Your Health Notts. This transformative service will play a central role in system change, bringing previously separate lifestyle behaviour change services together into one holistic service. It seeks to work collaboratively with communities to support the transformation of services for the local population. The service will provide integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity and falls prevention, all embedded in sustainable behaviour change and positive wellbeing.
Mainduties of the job
Your key responsibilities will include delivering ABL Healths family weight management programme through group and one-to-one sessions in the community and via digital platforms. You will implement healthy lifestyle education in schools, support families with toddlers experiencing selective eating, and deliver nutrition and physical activity sessions in various settings. The role requires a non-judgemental, client-focused approach, using motivational interviewing and evidence-based guidance.
You will contribute across all integrated services, including adult weight management, alcohol reduction, falls prevention, and smoking cessation, while managing caseloads, monitoring client progress, and conducting assessments. Additional duties include supporting safeguarding processes, coordinating resources, maintaining accurate records, and promoting SMART goal setting. Youll also engage in community outreach, stakeholder meetings, service development, and social media support. Ongoing responsibilities include attending team meetings, contributing to performance reports, developing resources, supporting new staff, completing required training, and traveling flexibly across the project area.

You will be a motivated, passionate, organised, and proactive advisor, with significant experience working in community health programmes. You will be passionate about promoting healthy lifestyles and delivering motivational change which is client centred. You will have a background in supporting people to build their confidence and ability to make sustainable healthy behaviour changes which works for their personal circumstances.
Our team have working backgrounds in nutrition, weight management, exercise, alcohol reduction, falls prevention and smoking cessation. We offer you a full in-house training package to help you support our clients on their journey to a sustainable healthy lifestyle change and you will be required to deliver on all elements of the integrated service when required.
Your key responsibilities will be:
Deliver ABL Healths family weight management programme including nutrition, behaviour change, and exercise delivery to groups and one-to-ones, in the community and through digital methods such as Zoom/phone appointments.
Implement and deliver ABL Healths healthy lifestyle curriculum to schools across Nottinghamshire.
Provide support and deliver a curriculum of activities to families with toddlers experiencing selective eating across Nottinghamshire.
Deliver nutrition and physical activity education to groups at extra-curricular clubs and events.
Deliver age-appropriate, evidence-based education on nutrition, physical activity, and behaviour change, following guidance from the ABL Health team and aligning with ABL Health programme curriculums.
