Main duties of the jobThe postholder will deliver engaging healthy ageing education sessions using the Older and Wiser framework, alongside strength and balance taster sessions that promote independence and falls prevention. Sessions will be tailored to community needs, with guest partners involved where appropriate, and will focus on building positive, trusting relationships while representing Your Health Notts in the community.
The role includes collecting and reporting key data and insights, maintaining accurate records, and contributing to partnership reporting requirements. You will create an inclusive environment for older adults, using person-centred approaches to support behaviour change and provide signposting to relevant local services.
Working collaboratively with statutory and VCSE partners, you will strengthen local pathways, attend community events, ensure safe session delivery through risk assessment, and contribute to ongoing programme development and continuous improvement.

Job responsibilitiesThe Community Ageing Well Programme is a new initiative developed in partnership with the Nottingham & Nottinghamshire Integrated Care Board. The programme aims to support older adults to remain independent, confident and socially connected by embedding healthy ageing education and strength & balance taster sessions within existing community groups across South Nottinghamshire.
The Older and Wiser guide forms the foundation of the educational offer, complemented by guest sessions from trusted partners including Nottinghamshire Fire & Rescue, Police Community Support Officers, NHS professionals, VCSE organisations, Adult Social Care and Social Prescribers.
Role Purpose
As the Community Ageing Well Practitioner, you will deliver engaging ageing-well sessions within established community groups, focusing on prevention, confidence-building and early support. You will build strong relationships, provide strength and balance taster sessions, coordinate partner involvement, and gather insight to support the programmes evaluation and wider system learning.
This role is essential in promoting independence, reducing loneliness, and helping older people feel more in control of their wellbeing.
Key Responsibilities
Programme Delivery and Community Engagement
Deliver accessible, engaging healthy ageing education sessions using the Older and Wiser framework.
Provide strength and balance taster sessions designed to support independence and falls prevention.
Tailor content to the needs and interests of each community group.
Coordinate guest sessions with partners (fire safety, fraud awareness, NHS professionals, etc.).
Build trusting relationships with community groups through proactive, friendly engagement.
Represent Your Health Notts as a positive and supportive presence in the community.
Insight, Data and Reporting
Capture pre- and post-session wellbeing measures and fall history checks.
Record session attendance, topic requests, participant feedback and engagement levels.
Contribute data and reports to the South Nottinghamshire Place partnership.
Participant Support
Create an inclusive and welcoming environment for older adults.
Use motivational, person-centred communication to support behaviour change and confidence.
Provide signposting to local support services including VCSE groups, social connection opportunities, lifestyle services and falls pathways.
