Stop Smoking Practitioner - Secondary Care
As a Tobacco Dependency Practitioner for Secondary Care, you will be an integral part of Smoking Cessation in hospitals, working alongside existing hospital-based Tobacco Dependence Teams to ensure a medical model of care for tobacco dependency is an integrated part of routine client care by all health professionals. Ultimately this programme will lead to the most significant referral route for the overall smoking cessation service within the county.
Training will be given to ensure staff / applicants are equipped to deliver effective services.
Main duties of the jobYour key responsibilities will include delivering smoking cessation and tobacco dependency support from hospital admission through to community follow-up. Youll embed cessation into routine care, providing one-to-one, group, and remote interventions in line with NICE and NCSCT guidelines. The role involves motivating clients to make informed decisions, setting sustainable goals, and managing a caseload. You'll collaborate with clinical teams and partner services, lead peer support groups, engage clients at community events, and maintain accurate records.
Youll also support staff development, contribute to service improvement, attend relevant meetings and training, and promote integrated, evidence-based care. Job responsibilitiesYour key responsibilities will be:
To be responsible forthe assessment, planning and delivery of a programme of smoking cessation /tobacco dependency care to clients at the hospital bedside and follow up seamlessly into the community on discharge
To support with embedding smoking cessation / tobacco dependency into routine medical care
Assisting clinical teamsin providing advice to help tobacco users to stop smoking or using other formsof tobacco.
Ensure tobacco dependency interventions you will be delivering comply with NICE, and NCSCTguidance.
Work cohesively withother services to ensure multi-agency working is integrated into tobacco dependency.
Identify opportunities to develop and improve services for clients.
Contribute to the development of secondary care staff.
Motivate and empower clients to make changes to address their addiction
Explain to clients the principles of the Stop Smoking Service to enable them to make informed choices
Deliver evidence-basedstop smoking interventions, these include one to one, drop in, group sessions,telephone, or on-line support, these must be in accordance with the NCSCTStandard Treatment Programme.
Work with other professionals to embed smoking cessation is part of routine client care
Maximise the impact ofsmoking cessation on health gains and reduce health inequalities
Lead peer support group sessions for successful quitters following intervention to prevent relapse
Provide evidence-based education in all aspects of smoking cessation and behavioural change as wellon- going motivational support
Work with individuals toset realistic sustainable goals
Ensure all client and service records are up to date, accurate and satisfy the ABL Health documentation policies Manage a caseload
To engage and recruit potential clients at promotional and community events
Provide administrative assistance to the service in terms of client contacts, appointment bookings and confirmations
Identify opportunities to develop and improve services for clients
Maintain own knowledge of evidence-based methods and integrate new research into own practice
Assist in the organisation of the workload ensuring effective and efficient communication toother staff members
To conduct stakeholder engagement, identify and strengthen referral pathways and identify potential co-working opportunities
Attend stakeholder meetings as required
Attend regular Stop Smoking Service Meetings
Attend training as required and keep up to datewith the wider aspects of Tobacco Control Attendall training, internal and external to support the role
You will be a motivated, passionate, organised, and proactive individual with significant experience in smoking cessation or addiction.
This role will be demanding as services develop andshould become integrated in all settings. It will be essential that you useyour own judgement to prioritise competing demands and workload effectively to specific timescales.
