Community Ageing Well Practitioner

Key Responsibilities

• Deliver accessible, engaging healthy ageing education sessions using the Older and Wiser framework.
• Provide strength and balance taster sessions designed to support independence and falls prevention.
• Tailor content to the needs and interests of each community group.
• Coordinate guest sessions with partners (fire safety, fraud awareness, NHS professionals, etc.).
• Build trusting relationships with community groups through proactive, friendly engagement.
• Represent Your Health Notts as a positive and supportive presence in the community.
• Capture pre‑ and post‑session wellbeing measures and fall history checks.
• Record session attendance, topic requests, participant feedback and engagement levels.
• Contribute data and reports to the South Nottinghamshire Place partnership.
• Create an inclusive and welcoming environment for older adults.
• Use motivational, person‑centred communication to support behaviour change and confidence.
• Provide signposting to local support services including VCSE groups, social connection opportunities, lifestyle services and falls pathways.
• Work collaboratively with Nottinghamshire Fire & Rescue, Police Community Support Officers, NHS partners, Social Prescribers, VCSE organisations and Adult Social Care.
• Strengthen community connections by promoting local support pathways and partnerships.
• Represent YHN at external events and partner sessions.
• Maintain accurate and timely records in line with organisational policies.
• Complete dynamic risk assessments and ensure safe delivery of all sessions.
• Support programme review and evaluation at project close.
• Stay up to date with healthy ageing, frailty and falls prevention evidence.
• Contribute ideas to enhance educational content and programme design.
• Participate in relevant internal and external training.

• Degree in Public Health, Exercise, Psychology, Nutrition or similar.

• Relevant experience in community health, older adult support, behaviour change or activity delivery.
• Experience delivering group sessions in community settings.
• Experience engaging older adults or vulnerable communities.
• Experience delivering strength & balance or falls prevention activity.
• Experience working with multi‑disciplinary teams.
