Service Coordinator

This is an exciting opportunity for an experienced care professional who is ready to take the next step in their career. As a Service Coordinator, you will play a key role in ensuring the delivery of safe, high-quality, person-centred care. You will coordinate and motivate a vibrant team of Care staff, ensuring excellent outcomes for our learners and citizens while maintaining compliance with regulatory requirements and standards.

You will:

• Lead and support a team of approximately 15 to 20 care staff.
• Deliver and oversee person-centred care planning and support.
• Ensure safeguarding practices and Mental Capacity Act principles are embedded within service delivery.
• Administer medication and PEG care in accordance with organisational procedures.
• Monitor and review learner and citizen progress against agreed goals and outcomes.
• Build effective relationships with families, advocates and healthcare professionals.
• Conduct staff supervision and annual performance reviews.
• Support recruitment, induction and workforce development activities.
• Contribute to quality assurance processes and continuous service improvement.
• Act as Site Coordinator on a rota basis.

We’re looking for someone who has:

• Excellent verbal and written communication skills.
• Experience in supporting people with learning disabilities, autism, physical disabilities and complex needs.
• Basic IT skills, including Microsoft Office and report writing.
• The ability to prioritise workloads and meet deadlines.
• Experience working effectively as part of a team.
• Strong interpersonal skills and the ability to build positive relationships.
• A demonstrable understanding of safeguarding practices, GDPR and confidentiality requirements.
• Knowledge of CQC Fundamental Standards.
• Experience of delivering staff training and coaching.

• Level 3 NVQ in Health and Social Care

Working Hours

This role operates on a flexible shift pattern across seven days:

• 6:45am to 2:45pm
• 2:00pm to 10:30pm
