Main Job Purpose To provide creative and flexible support to meet the needs of rough sleepers to exit the street and street based lifestyles, and those at risk of rough sleeping into successful sustained resettlement.

Job Duties and Responsibilities
Service Delivery The aim of the Rough Sleeping Navigators Service is to assist individuals who are new, entrenched, return, repeat or housed rough sleepers into assessment, support and accommodation.
Casework Management To be responsible and accountable for providing advice, support, motivation, crisis intervention and future planning, appropriate to the expressed needs of beneficiaries with the aim of ensuring that beneficiaries live sustainably and independently is secure accommodation.
To be responsible and accountable for supporting a defined number of rough sleepers in Nottingham City who have complex and low level needs, enabling them to exit rough sleeping into safe, secure accommodation through a range of activity including:
swift identification of people sleeping on the streets
information gathering for referral for further assessment
flexible and persistent contact and communication to maximise engagement
coordination of wraparound support from specialist services to ensure that wider needs are met and are not barriers to accepting offers of support and utilising offers of accommodation
retaining a consistent single point of contact for individual and services until rehousing is achieved
Organisational Responsibilities To be responsible and accountable for contributing to the successful delivery of Framework's services for vulnerable and homeless people.
