** Clinic RN
** accurately and appropriately provides patient-centered care by means of health protection, promotion, optimization and prevention of illness or injury. This role performs both independent and delegated nursing functions including: assessment, nursing diagnosis, planning, implementation and evaluation.
** What you will do as a Clinic RN II**:
* Establishes measurable and achievable short- and long-term goals for patients
* Implements nursing protocols based on the treatment plan, ensuring continuity of care for patient. Provides education to patient and caregiver(s)
* Evaluation of the patient’s status and efficiency of the nursing care and modified care plan as needed
* Helps provide healthcare access through nurse visits, management of in-basket task (phone messages, MyChart communication, refill requests, results)
* Provides safe and timely telephonic triage for symptomatic callers, using system approved triage policies and protocols##

* What you will need:

*** Current Minnesota RN Licensure and knowledge of specialty area
* Associate’s (ADN) or Bachelor’s Degree in Nursing (BSN)
* Healthcare Provider Basic Life Support (BLS)
* Excellent assessment and care coordination skills

Casual RNs are shared between both of our Maple Grove and Wyoming sites to cover sick time, vacation, FMLA, etc..
