Position Summary

Good afternoon. I wanted to relay the expectations for attendance and tardiness. Per Minute Clinic’s attendance policy there is no “grace period” for tardiness. During our recent Area 4 meeting, management and I learned this per HR. If your shift begins at 8:30, you will be considered tardy at 8:31. Management and I will begin to closely monitor attendance and tardiness beginning March 1st, 2024.

As expressed, 1 tardy=0.5 occurrence and 1 unexcused absence=1 occurrence. Once you hit 5 occurrences in a 12-month period, you receive a level 1 Performance Plan. All worked days, including staff meeting, count toward attendance.

I highly recommend arriving 5 min prior to your shift to log into your computer, clock in, complete myWork task, and huddle with your providers. You will be paid for the 5-minute arrival prior to your shift.

Also, please remember to take your full lunch hour/half-hour. You are expected to work up until your lunch clock out time and be ready to see patients immediately after clocking in from lunch. After clocking in from lunch, your clinic door should be open or you should be seated at the desk if your clinic is participating in the PCA Pilot, ready to see patients.

If it appears that you may work past your end of shift time, work up the last patient, and ask the provider if they need you to stay for exams, labs, etc. If the provider does not need you to stay to assist, please clock out. If you must stay after your end of shift to assist the provider, please email me and the SPM to notify.

The SPM and I need to understand why there is a delay clocking out and see how we can assist with this issue.

We need to be very observant of our clock in and out times and not going into overtime or working longer than our scheduled shift times. I am very hopeful that attendance and tardiness will not be an issue.

Required Qualifications
  • Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
  • Administer immunizations and injections (to include but not limited to COVID
    19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
  • Minimum of one-year related work experience in a healthcare setting
  • Experience documenting within an Electronic Medical Record
  • Ability to take vital signs, perform phlebotomy blood draws and perform Point of Care Testing
  • Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
  • Pass a FIT test and wear the provided respirator mask when protocol is required.
Preferred Qualifications
  • Phlebotomy Certificate or phlebotomy experience preferred. If not experienced, must successfully complete internal phlebotomy training.
  • Previous experience in Primary Care, Urgent Care or Emergency Room
Education
  • Graduate of a school of Practical or Vocational Nursing, approved by the State Agency and/or accredited by the National League for Nursing (Accrediting Commission (NLNAC)) at the time the program was completed by the applicant.
Licensure/Certification s
  • The LPN/LVN must have a full, active and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States or District of Columbia.
