Looking for flexibility, a great rehab team, and the opportunity to make a difference? Phoenix of Maple Heights is looking for a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team!

As a PRN PTA, you’ll work alongside our Physical Therapists and interdisciplinary team to help residents improve mobility, strength, function, and independence.

Responsibilities include:

• Provide physical therapy treatments according to the established plan of care
• Work closely with the supervising Physical Therapist
• Help residents achieve their functional and mobility goals
• Accurately document treatments and resident progress
• Communicate with the rehab and interdisciplinary teams
• Provide compassionate, resident-centered care

• Current Ohio Physical Therapist Assistant license
• Graduate of an accredited PTA program
• SNF/LTC experience is a plus
• Experience with Net Health is a plus
• Dependable, positive, and team-oriented
• Strong communication and documentation skills
