Registered Nurse, Transition Line (Emergency Specialty Nursing) – Ridge Meadows Hospital
Salary: CAD $41.42 – $59.52 per hour.

OverviewThis role provides registered nurses the opportunity to consolidate their skills by rotating through a specialty nursing unit(s) prior to and/or during the completion of post‑graduate specialty nursing courses. Successful applicants will participate and complete applicable post‑graduate specialty nursing courses/program within agreed time frames.
Responsibilities- Assess patients, collect information from a variety of sources and make nursing diagnoses.
- Collaborate with patients and families in developing care plans and encourage family participation.
- Participate in care and discharge planning, ensuring plans are implemented and evaluated effectively.
- Coordinate nursing care, set priorities, delegate tasks and make referrals according to protocols.
- Collaborate with interprofessional team members to resolve issues and adjust care plans.
- Teach patients and families about illness prevention and health maintenance.
- Plan and discuss referral information with patients, families and interprofessional team.
- Document assessments, diagnoses, observations, interventions and referrals according to standards.
- Participate in nursing committees, care conferences and councils as assigned.
- Participate in research and quality improvement activities.
- Attend in‑services and other educational programs to maintain competency.
- Provide feedback to the manager regarding effectiveness of patient care programs.
- Perform other related duties as required.
Graduation from an approved School of Nursing with current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Completion of applicable post graduate specialty nursing course/program and willingness to complete future courses. Current certification in Basic Cardiac Life Support (BCLS).
Skills & Abilities- Complete initial and ongoing patient assessment and provide nursing care through appropriate interventions.
- Communicate effectively with patients, families and interprofessional team using verbal, written and electronic means.
- Promote person‑centered care with empathy and sensitivity to diverse cultures.
- Integrate and evaluate data from multiple sources to problem‑solve effectively.
- Teach and instruct patients and families on essential topics for health care and well‑being.
- Organize and prioritize work.
- Collaborate with and promote cooperation among interprofessional team members.
- Use factual information and evidence‑based practice to support nursing decisions.
- Operate related equipment and computer applications.
- Have physical ability to perform the duties of the position.
