Diabetes Management Nurse Position

Drive excellence in diabetes management as a Chronic Disease Nurse. Focus on patient education and self-management strategies to foster healthier lifestyles in individuals with diabetes.

This role involves creating and evaluating care plans tailored to diabetic patients and sharing expert knowledge within a cohesive team. Your contributions will include patient assessments and mentoring students and new staff in best practices and policies for diabetes care.

Key Responsibilities:

• Design and implement personalized diabetes care plans

• Facilitate patient education and support programs

• Maintain accurate documentation of patient care

• Provide mentorship to nursing staff and students

• Engage in quality improvement projects

Requirements:

• Graduation from a recognized Nursing program

• At least 2 years of relevant diabetes management experience

• Certification as a Diabetes Educator preferred

• Valid registration as a Nurse in BC

• CPR certification necessary
