As a Patient Care Coordinator, you will supervise nursing staff and ensure daily operations run smoothly, focusing on patient discharge planning and care paths.

Your role includes promoting best practices, mentoring staff, and participating in quality improvement activities to maintain high standards of healthcare delivery.

Key Responsibilities:

• Foster collaboration among healthcare professionals

• Oversee admissions, patient flow, and discharges

• Mentor nursing staff and assess their performance

• Facilitate care conferences and communications

• Manage quality improvement and risk management activities

Requirements:

• Current registration as a Registered Nurse in British Columbia

• At least 3 years of clinical experience with some supervisory

• Completion of relevant post-graduate coursework

• Strong ability to prioritize and manage patient care

• Excellent communication and leadership skills
