Salary CAD $42.27 - $52.81 per hour

Requirements
Bachelor’s degree in Social Work
At least 2 years of recent related clinical social work experience working with individuals, groups and families
Current full registration with the BC College of Social Workers
Detailed Overview
Reporting clinically to the Professional Practice Chief, Social Work and administratively to the Manager, Health Services, the role involves providing social work services to improve the emotional and social consequences of a health condition for the client and family. It includes promoting effective coping, encouraging optimal functioning, and ensuring safe and timely discharges.
Responsibilities
Identify and engage eligible clients using various screening and case‑finding techniques.
Conduct social and psychosocial assessment of patients and families and interpret the information using appropriate modalities.
Provide therapeutic counseling services for clients and families experiencing a range of illness‑related circumstances.
Facilitate the process whereby patients and families identify their needs for recovery, adaptation, and mastery, and promote client/family autonomy in planning and decision‑making.
Follow up and provide ongoing monitoring of psychosocial situations, evaluating responses to interventions and revising care plans as necessary.
Act as a resource for clients and families by providing information on community resources, facilitating referrals, and collaborating with other professionals.
Help clients explore reactions to loss and provide counseling from a strengths‑based perspective.
Coordinate and advocate for clients with other service providers to promote maximum social functioning.
Participate in quality improvement activities and help develop, implement, and evaluate program goals, objectives, policies, and procedures.
Educate other health service providers about psychosocial aspects through discharge planning rounds and conferences.
Document clinical interventions and participate in outcome assessments and workload measurement activities.
Perform other related duties as assigned.
Qualifications
Education and Experience
Bachelor's degree in Social Work from an approved school of Social Work plus two (2) years of recent related clinical social work experience. Current full registration with the BC College of Social Workers.
Skills and Abilities
Demonstrated knowledge and understanding of counselling skills and clinical practice models for individuals, families, and groups.
Ability to conduct social and psychosocial assessments.
Leadership in interdisciplinary team care planning in complex client situations.
Effective collaboration in a multicultural environment.
Understanding of the scope of practice of other health care disciplines.
Proficient referral-making, with in‑depth knowledge of community resources.
Excellent verbal and written communication with clients, families, team members, and community agencies.
Strong conflict resolution skills.
Facilitation of group meetings, processes, and decision‑making.
Ability to operate related equipment and perform duties physically as required.
