The salary range for this position is CAD $49.20 - $67.08 / hour
Job Summary
As Client Care Coordinator, supervise and provide work direction to staff, coordinate staffing coverage, provide clinical leadership, guidance and support to the interprofessional team, and act as a clinical resource. Assist staff to provide quality care by offering specialized knowledge, assessing clinical skills, and supporting client care activities as needed.
Responsibilities
Provides leadership to the interprofessional team by coordinating client care activities, providing information and consultation to designated care providers and students, reviewing client and family needs or progress, troubleshooting, and guiding management of clients.
Supervises and provides work direction to staff through scheduling, coordinating and adjusting work activities according to unit needs, establishing priorities, and giving performance feedback.
Determines related training and orientation requirements, establishes team goals and objectives, and contributes to staff selection and performance evaluation.
Evaluates care provision by observing staff, reviewing documentation, interviewing patients and families, investigating incident reports and providing interventions.
Participates in safety program maintenance, identifying and recommending corrective actions for safety concerns.
Coordinates quality improvement and risk management activities, evaluates practice changes, conducts safety audits, tracks incidents, and recommends practice changes.
Participates in staff development, conducts needs assessments, supports new skills introduction, and oversees orientation and progress of new staff.
Oversees quality assurance, reviews client records for accuracy, and recommends program effectiveness improvements.
Consults and collaborates with team members to resolve client/family issues, ensures documentation, identifies resources, and proposes solutions.
Participates on committees and task groups to resolve issues and support program goals; liaises with other programs, agencies and hospitals.
Promotes research-based practice by encouraging staff awareness, identifying research needs, and supporting ongoing projects.
Assists the Manager in planning, developing and evaluating client care, reviewing policies and procedures, and recommending clinical and administrative practices.
Provides clinical leadership and consultation to assess, plan, implement and evaluate care, ensuring compliance with standards and fostering collaboration.
Assists the Manager with administrative activities, optimizes resource use, informs budget and staffing decisions, and recommends process changes.
Provides consultation or screening of clients in special circumstances, identifies and responds to client care issues and emergencies, and evaluates admission/discharge needs.
Sets measurable performance goals, maintains professional knowledge in Mental Health and Substance Use, and collaborates with Managers for development planning.
Assists staff with client care activities as required.
For licensed facilities, may oversee building maintenance requests, develop health and safety plans, and ensure compliance with licensing requirements; respond to inspections.
Performs other related duties as assigned.
Qualifications
Education, Training and Experience
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the BCCNM. Three (3) years of recent related clinical experience in the designated area, including one (1) year in a supervisory role, or an equivalent combination of education, training and experience. Completion of a substance use certificate program as required. For licensed facilities: completion of St. John's Ambulance Safety Oriented First Aid Certificate and current CPR level C or equivalent.
For MICR only:
Enhanced Security Police Clearance.
Skills and Abilities
Clinical proficiency/expertise in the designated clinical area.
Knowledge of community resources and health-related support services.
Ability to supervise, lead and direct staff.
Ability to coordinate client assignments in collaboration with staff.
Ability to monitor staff workload and patient factors and adjust assignments accordingly.
Ability to establish priorities for client care, patient flow, discharge planning, and daily unit operations.
Ability to monitor quality of care and ensure maintenance of standards for nursing practice and support staff.
Ability to monitor performance, address issues based on professional standards, and provide input for performance reviews.
Ability to role model, coach and mentor best practices in patient care.
Ability to identify learning priorities and support implementation of education based on changing practices.
Ability to provide input into budgets and equipment decisions.
Strong verbal and written communication skills.
Ability to work independently and self-directed.
