Responsibilities
  • Provides direct nursing care to clients receiving scheduled or urgent/timely primary care; assesses clients and collects information from a variety of sources; makes a nursing diagnosis and determines appropriate nursing interventions or collaboration with other clinical staff.
  • Processes physician or nurse practitioner orders by initiating unit standing orders, administering medications, carrying out treatments and monitoring effects.
  • Establishes a therapeutic relationship with the client to support autonomy in decision-making and care planning while respecting dignity and privacy.
  • Screens referrals, provides individualized client assessments, inter-professional care planning and interventions; coordinates care with primary care providers, specialists and community resources to optimize outcomes.
  • Develops a comprehensive client care plan in collaboration with the inter-professional team and facilitates the transition to primary/community care providers or community agencies.
  • Delivers planned and unplanned client care and explains care appropriately to clients and families; ensures access to required services.
  • Reviews and updates client care plans with the inter-professional team, initiates referrals as needed, and plans next steps across the care continuum.
  • Discusses clinical findings with client/family and develops action plans aligned with clinical needs and self-management goals; prioritizes care and manages resources effectively.
  • Supports clients and families during clinic visits, provides information to aid decision-making, and advocates for client needs.
  • Organizes outreach nursing care to clients in home settings or suitable alternatives when required; provides plan of care and information about community resources.
  • Provides health counselling, education, self-management coaching and wellness promotion through clinic, telephone or home visits; assesses learning needs and evaluates outcomes.
  • Maintains clinical records and adheres to policies, standards and procedures; participates in quality improvement activities and professional development.
  • Presents input in the development and revision of standards of care, policies and procedures; contributes to quality improvement initiatives.
  • Performs other related duties as assigned.
Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

One (1) year recent related clinical experience in acute care, urgent/emergency care or related clinical area including assessing and treating children, youth and adults in an acute or community/outpatient setting, or an equivalent combination of education, training and experience.

Valid BC Drivers license and access to a personal vehicle for business related purposes, as required.
