Registered Nurse, Patient Assessment and Transition to Home (PATH) – Ridge Meadows Hospital – Maple Ridge
Salary: CAD $41.42 – $59.52 per hour.
Job SummaryWe are looking for a casual Registered Nurse to deliver high‑quality care to patients while assessing their physical and psychological needs. The role focuses on providing individualized care plans, facilitating discharge planning, and ensuring a safe, therapeutic environment.
Responsibilities- Assesses, plans, implements and evaluates nursing care for patients or groups, identifying problems and collaborating with the interprofessional team.
- Collaborates and consults with patients, their families and the interprofessional team regarding the patient care plan, encouraging participation and resolving issues.
- Provides teaching to patients and families, identifying health‑related learning needs and referring to appropriate resources such as respiratory therapy, home health or palliative care.
- Maintains patient records, documenting nursing observations, interventions and outcomes; reports significant changes to the physician or manager.
- Initiates discharge planning upon admission, intervenes as needed, and advocates for patient autonomy, dignity and access to information.
- Maintains a safe and therapeutic environment, adhering to infection control, fire and emergency procedures.
- Participates in quality improvement activities, collecting data and recommending changes to improve patient care.
- Precepts nursing staff and students during orientation and ongoing education.
- Utilizes resources efficiently and responsibly in patient care delivery.
- Serves on nursing and hospital committees, interprofessional conferences and councils to enhance patient care.
- Performs other related duties as assigned.

Education and Experience: Graduation from an approved nursing school with current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities- Ability to assess patients and apply assessments to determine appropriate care interventions.
- Ability to develop and implement comprehensive care plans.
- Knowledge of chronic management models.
- Ability to teach and promote chronic disease self‑management.
- Effective verbal and written communication skills.
