Responsibilities- Facilitates timely patient placement at acute sites and designated Fraser Health sites; assesses current and projected capacity, prioritizes and expedites critical issues; leads daily bed meetings with Patient Care Coordinators and site leadership.
- Consults with the Patient Transfer Network (PTN) and Community Resources to identify appropriate care, understand available services, determine contacts, and discuss bed availability and resources on a patient-to-patient basis.
- Coordinates acute site transfer planning including arranging patient/resident/client (P/R/C) and/or equipment transfers, both external and internal; identifies and reports potential discharges and communicates with the Manager.
- Advises FH leadership, acute and community staff on the interpretation and application of patient access and flow policies and eligibility for non-acute bed resources; assesses critical situations and recommends action to optimize bed utilization and patient placement.
- Supervises and coordinates clinical staff after hours when service leaders are unavailable; coordinates equipment and resource use and directs staffing adjustments as needed.
- Acts as a clinical resource for nursing staff on patient placement and bed utilization; assists Bed Booking staff in allocating beds and liaising with nursing units, reporting issues to the Manager.
- Demonstrates clinical expertise, mentors staff, and collaborates with the interdisciplinary team to optimize timely bed access and patient placement; develops and recommends policy changes as appropriate.
- Analyzes statistics and trends (e.g., ALC, ALOS/ELOS, Acute Dashboard) and participates in strategies to manage designated patient populations; collaborates with clerical staff to maintain a bed availability database.
- Investigates incidents related to patient or staff safety in line with applicable policies and quality improvement protocols; documents and forwards reports to the Manager.
- Communicates with physicians, families, and departments regarding nursing practice and hospital policy; escalates unresolved issues to the Manager for follow-up.
- Monitors nursing practice quality and patient safety through assessment; identifies quality improvement opportunities and communicates findings to the Manager.
- Participates on internal committees to provide an organizational perspective on patient placement and bed utilization; shares updates on issues, trends, and opportunities for improvement.
- Notifies the administrator on call of any safety, legal, or media implications; initiates Emergency Preparedness actions as needed.
- Performs other related duties as required.
Education and Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Five (5) years of recent, related clinical experience in an acute care, multidisciplinary facility including nursing leadership experience; preference for experience in critical care areas or access and planning, or an equivalent combination of education, training and experience.
