Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Coordinator is responsible for the day‑to‑day operations of the unit(s), including daily patient flow, and supports research, policy, quality improvement and risk management activities.
Responsibilities
Apply established criteria and prioritize admissions/intake, lead and facilitate daily patient flow actions, plan for patient discharge and appropriate placement/referral by ensuring the care team follows through on care paths and processes.
Supervise nursing and nursing support staff, coordinate clinical team work on a day‑to‑day basis, observe work, correct deficiencies, monitor quality of care and patient safety, assign and adjust work assignments, schedule staff, authorize staff hours and ensure staffing requirements are met.
Support quality patient care and excellence in practice by promoting interprofessional collaboration, team building, staff education, mentoring, role modeling, assessing clinical skills, coaching staff and liaising with other health professional leaders to achieve clinical performance goals.
Initiate or lead plan‑of‑care conferences and rounds (e.g., iCare) for optimal patient care management, disseminate clinical information to nursing/nursing support staff and other team members.
Identify staff learning requirements, collaborate with the Manager, Educator and other team members to develop learning objectives, provide clinical support and evaluate effectiveness of learning opportunities.
Contribute to the development of a comprehensive orientation program in collaboration with the Educator and other clinical staff.
Act as point person for patients and families with concerns or questions, follow up as required, participate in or lead investigations of complaints, implement corrective action and collaborate with Quality Improvement according to established protocols.
Participate in quality improvement and risk management activities such as evaluating practice changes, generating recommendations, conducting safety audits and supporting implementation of teaching or learning initiatives.
Assist the Manager/Director with panel interviews, staffing recommendations, performance appraisals, budgetary resource use and equipment needs.
Promote staff awareness and involvement in research, evaluation activities and special projects; participate in research and special projects as appropriate.
Collaborate with internal and external stakeholders, serve on professional or operational committees at the unit, hospital, program and/or regional level.
Participate in review, development, implementation and evaluation of evidence‑informed clinical decision support tools/policies.
Perform other related duties as assigned.
