Key Responsibilities   Conduct thorough patient assessments upon admission
Create and evaluate individualized nursing care plans
Coordinate discharge planning with care teams
Instruct patients and families on health management

Requirements   Graduation from an accredited nursing program
Current registration with BCCNM
At least one year of related acute care experience
Effective communication skills across care teams
Capacity for performing nursing interventions safely
