Community Health Registered Nurse Position

Location: pitt meadows

Take on a vital role as a Community Health Registered Nurse in Maple Ridge, B.C. Use your nursing expertise to provide care and support for community members experiencing health challenges.
In this position, you will work autonomously within the community while collaborating with an interprofessional team. Responsibilities include comprehensive client assessments, personalized care planning, and providing direct care services to enhance patient health outcomes. By advocating for your clients and facilitating access to resources, you will help families navigate their healthcare journeys confidently and effectively.

Key Responsibilities:

• Conduct valuable client assessments and referrals

• Develop individualized care plans in collaboration with the team

• Deliver nursing care and education to clients

• Maintain patient records according to policies

• Coordinate partnerships with other health professionals
Requirements:

• Registration with BC College of Nurses and Midwives

• Minimum one year of relevant clinical experience

• Valid BC Driver's license for business use

• Knowledge in geriatric care and chronic conditions

• Strong problem-solving and communication skills
