We are looking for a skilled Licensed Practical Nurse to join our dynamic team at Ridge Meadows Hospital. In this role, you will assess and address the physiological, psychological, socio-cultural, and spiritual needs of patients. Your contributions will include developing individualized care plans and collaborating in multidisciplinary meetings.

Key Responsibilities:

• Assess patients and families to determine care needs

• Provide support and promote healthcare decision involvement

• Contribute to care plans and attend planning meetings

• Deliver nursing care per policies and evaluate outcomes

• Maintain equipment and ensure a safe environment

Requirements:

• Graduate from a recognized practical nursing program

• Current registration as a Licensed Practical Nurse with BCCNM

• Effective verbal and written communication skills

• Ability to organize work and implement care plans

• Knowledge of full scope of practice including medication administration
