• Assesses patients and collects information from a variety of sources, plans, performs, and evaluates nursing interventions.
• Collaborates with patients and their families/care givers, encourages family participation, establishes a therapeutic relationship, advocates for patient rights, and ensures patient autonomy.
• Participates in care and discharge planning, collaborates with the interprofessional team, and communicates with the Patient Care Coordinator.
• Coordinates nursing care for the patient; sets priorities, assigns and delegates tasks, and makes referrals according to established protocols.
• Collaborates with physicians, licensed practical nurses, patient care coordinators, clinical nurse educators, and other health care professionals to resolve issues and adjust care plans.
• Teaches patients and families about illness prevention, health restoration, and maintenance, assessing learning needs and providing information and demonstrations.
• Plans and discusses referral information with patients and families and initiates referrals to appropriate health care/community agencies.
• Documents assessments, nursing diagnoses, observations, interventions, outcomes, and referrals in accordance with practice standards.
• Participates on nursing committees, care conferences, and councils as assigned.
• Participates in research and quality improvement activities, identifying patient care issues and proposing corrective actions.
• Participates in in‑services and other educational programs, identifying learning goals and maintaining clinical competence.
• Acts as a preceptor or mentor to assigned new nursing staff and students, sharing information and participating in orientation.
• Provides feedback to the manager regarding effectiveness of patient care programs and policies.
• Performs other related duties as required.

Education and Experience

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
