Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and Fraser Health Mission and Values, the Registered Nurse provides direct patient care in an acute rehabilitation unit by assessing, planning, providing and evaluating nursing care.
Responsibilities
Assesses patients in an acute rehabilitation setting, collects information from multiple sources, and plans, performs and evaluates nursing interventions and care.
Collaborates with patients and families in developing and updating the care plan; fosters a therapeutic relationship and ensures patient privacy, dignity and autonomy.
Participates in admission and discharge planning; communicates with the interprofessional care team to ensure coordinated care.
Coordinates nursing care, sets priorities, delegates tasks to support staff, and makes referrals according to standards.
Works with the interprofessional team to identify and resolve issues, adjust care plans as needed, and respond to changes in patient condition.
Educates patients and families about illness prevention, health restoration and maintenance; assesses learning needs and supports self-care and discharge planning.
Initiates referrals to appropriate health care or community services in collaboration with the care team.
Documents assessments, diagnoses, interventions, patient responses, outcomes and referrals per standards and Fraser Health procedures.
Participates in nursing committees, care conferences and quality improvement activities; maintains ongoing clinical competence.
Acts as a preceptor or mentor to new nursing staff and students; participates in orientation.
Performs other related duties as required.
Qualifications

Education and Experience
Graduation from an approved School of Nursing with current registered nursing registration with the BCCNM. One (1) year of recent related acute care experience, or an equivalent combination of education, training and experience.

Skills and Abilities

Ability to perform initial and ongoing patient assessments and provide nursing care with appropriate interventions.
Knowledge of rehabilitation nursing in an acute care setting.
Effective communication with patients, families, the public, medical staff and the interprofessional care team.
Ability to promote person-centered care with empathy, cultural sensitivity and patient advocacy.
Ability to organize, prioritize and collaborate with the interprofessional team.
Ability to use evidence-based practice to support nursing decisions.
Ability to operate related equipment and computer applications.
Physical ability to perform the duties of the position.
