In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person‑centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute rehabilitation unit.

• Assess patients within an acute rehabilitation setting, collect information from a variety of sources, make a nursing diagnosis, and determine whether the condition can be improved by appropriate nursing intervention. Plan, perform, and evaluate nursing interventions and care.
• Collaborate with patients and their families/caregivers in developing the care plan, encouraging family participation, and ensuring patients’ rights to privacy, dignity, and access to information.
• Participate in the care and discharge planning process, ensuring the care plan is established and implemented effectively, updating the plan and intervening as appropriate.
• Coordinate nursing care, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols.
• Collaborate with members of the interprofessional care team—including Physicians, Licensed Practical Nurses, Patient Care Coordinators, Clinical Nurse Educators, and other health care professionals—to identify and resolve issues, adjust care plans, and respond to changes in patient condition.
• Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance, assessing learning needs and providing information and demonstrations of care activities.
• Plan and discuss referral information with patient and family, and initiate referrals to appropriate health care/community agencies.
• Document assessments, nursing diagnoses, observations, interventions, outcomes, and referrals according to practice standards and Fraser Health procedures.
