Community Health Registered Nurse

Deliver community-based nursing care in Maple Ridge with assessment and chronic disease management skills. This Registered Nurse role serves clients at home and in community settings.
Fraser Health seeks a Community Health Nurse working independently and within an interprofessional team. You will assess complex adult and geriatric clients, coordinate care, provide therapeutic interventions, and support transitions across healthcare services. Key Responsibilities:

Assess clients with complex chronic health conditions
Develop shared care plans with interprofessional teams
Provide direct care, counseling, and health education
Coordinate transitions across primary and community care
Maintain clinical records and statistical information
Requirements:
Current BCCNM practicing registration as a Registered Nurse
One year of recent related clinical experience

Experience with chronic conditions and care planning
Valid British Columbia driver’s license and personal vehicle
Ability to work independently and collaboratively
Use assessment, care coordination, and client education skills as a Community Health Registered Nurse with Fraser Health.
