Indigenous Social Worker

Position: Indigenous Social Worker at Fraser Health
Location: Pitt Meadows

Make a real impact at Fraser Health as an Indigenous Social Worker in Maple Ridge/Pitt Meadows, B.C. Support First Nations, Métis, and Inuit individuals with tailored mental health services.

In this part-time role, you will bring your social work expertise and cultural knowledge to an interprofessional team. Your responsibilities include conducting assessments, implementing care plans, and bridging clients to vital services. A focus on community resources and strong communication skills will be key.

Key Responsibilities:

• Assess and address psychosocial health care needs

• Develop and track recovery care plans

• Assist with client transitions from hospitals

• Provide consultation and support to clients and families

• Teach and share cultural insights with colleagues

Requirements:

• Bachelor’s degree in Social Work

• Experience working with Indigenous populations

• Active registration with BC College of Social Workers

• In-depth understanding of Indigenous health issues

• Ability to navigate the health system for clients
