Purpose

The Youth Services Case Manager (YSCM) is responsible for providing youth case management to youth in ENS’ Emma’s Place Family Program, as well as planning and implementing positive youth programming onsite. The YSCM will support youth with determining and addressing goals, work closely with other staff to ensure effective team communication that promotes client satisfaction, and provide effective services using a client-centered approach.

The typical schedule for this position is Monday – Thursday, 8am – 6pm.

Responsibilities
  • Provides on-site case management to youth at Emma’s Place and in the community
    • Tailors case management contacts to promote success and stability through assessing service needs, develops initial goal plans and coordinates services for youth residents alongside parents within 30 days of participant entry, and in six months increments thereafter
    • Assists clients in accessing community resources, school support, employment, criminal justice contacts, and accompany clients to initial and/or significant appointments and meetings relevant to their goals/services
    • Maintains up-to-date and organized client files through an electronic records system; including documentation of service plans and goals, case notes and program participation
    • Participates in client intake and move-in processes as needed, focuses on recovery and client-centered care, assists families in creating and following harm reduction plans, mediating conflict, and resolving crises as appropriate
    • Assists with covering the front desk as needed
  • Fosters a positive, engaged youth community through programming and community collaboration
    • Plans and provides effective school year and summer academic and social programming
    • Supports incorporating individual and group strategies to develop lesson plans and skill development exercise for school-age children
    • Develop schedule and logistics of program components delivered by volunteers and community partners
    • Attend on and offsite out of school time events and functions including family nights and student organized events
  • Functions as a team member
    • Collects and records all required statistical information, accurately reporting data in a timely manner
    • Follows through with own job responsibilities and assignments
    • Engages in constructive problem solving and conflict resolution, assists other team members in doing the same
    • Provides information needed by other team members in a timely and effective manner
    • Offers assistance to team members as needed
    • Gives and receives feedback to and from team members
    • Takes direction from supervisor or manager, and implements team decisions as made
