We are currently recruiting for a further Lead Reablement worker based full time in March, Cambridgeshire.

The Reablement Service provides care at home to people aged 18 upwards. Support is delivered in an enabling way so that people can maximise their independence. Reablement support is a short term service.

Lead Reablement Workers are responsible for coordinating and managing a complex caseload of service users at different stages of their reablement journey. They use professional judgement to assess risk, prioritise interventions, and adapt support plans to ensure individuals achieve the best possible outcomes. A key focus of the role is preventing unnecessary dependence on ongoing care services by maximising independence, confidence and self-management skills.

Appointment to these posts will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service. The Lead Reablement Worker plays a key role in delivering short-term, person-centered support that helps individuals regain confidence and independence in their daily lives. This role involves working closely with service users, their families, and other professionals to create tailored reablement plans and risk assessments that promote recovery and reduce reliance on long-term care.

Your day-to-day responsibilities will include:

• Assessing individual needs, completing risk assessments, and developing tailored reablement plans that focus on recovery, independence and positive outcomes.
• Managing and prioritising a diverse caseload, ensuring service users receive timely reviews, interventions and support in response to changing needs and levels of risk.
• Working hands-on with service users, helping them regain confidence and relearn essential daily living skills such as personal care, mobility, meal preparation and community access.
• Monitoring progress and reviewing plans, adapting goals and interventions to maximise independence and reduce reliance on long-term care and support services.
• Identifying and managing risks, balancing empowerment and independence with the need to ensure safe and effective support.
• Collaborating with Occupational Therapists, health and social care professionals, families and community services to provide a coordinated approach that promotes sustainable independence.
• Maintaining accurate records and documentation to ensure compliance, quality standards and effective case management.
