Reporting to the Lead Nurse for Falls, the post holder will be an expert Clinical Nurse Specialist and be able to demonstrate specialist clinical competence and knowledge base. With the support of the Lead Nurse, the post holder will lead the Falls Service at the designated work base, whilst providing team support at other sites as required.
Responsibilities- Provide specialist clinical advice to healthcare professionals supporting the approximate 1,818 yearly falls reported Trust Wide.
- Lead site-based falls leadership and provide specialist professional advice to nursing and multidisciplinary teams.
- Assist with report writing, present to committees of interest, and provide day‑to‑day support with patient assessment and proactive falls prevention.
- Embed improvement initiatives and develop new opportunities for improving the Falls Service and quality of patient care.
- Support the learning and development of all staff and pre‑registration learners on practice placement within the clinical area, including members of the multidisciplinary team.
- Facilitate a conducive learning environment to support best practice and safe patient care.
- Engage with the mandatory annual appraisal process, identifying individual learning needs and opportunities to support PDP.
- Ensure all relevant e‑learning, mandatory, statutory and essential training is completed and compliance is maintained.
- Engage in opportunities that expand knowledge, such as utilising best practice in the clinical area and sharing updates with the wider team.
- Maintain professional revalidation, self‑reflective practice and accurate capability records, and produce originals to demonstrate skills acquired.
- Manage time effectively and identify priorities for care delivery and Falls Service caseload.
- Maintain standards of the Falls Service as defined by guidelines, protocols, procedures and codes of conduct.
- Respond to and adapt to changes in needs and/or environment, supporting planning to use staff and resources effectively.
- Refer to and work with multidisciplinary teams to develop and support the service user, family and carers, providing specialised advice on complex patient care and management.
- Use numerical, literacy, digital and technological skills to support safe and effective delivery of care.
- Carry out systematic, evidence‑based assessments to provide holistic care, considering all relevant factors that influence the service user’s pathway or treatment.
- Use interprofessional and interpersonal skills to communicate complex, sensitive, and potentially contentious information to patients, families, carers, members of the public and workforce.
- Accept accountability and responsibility for delivery of care and decisions within clinical practice, ensuring all care is delivered within scope of practice.
- Maintain accurate, timely and legible records of documentation and care delivered, including consent and capacity information.
- Support staff to deliver safe management in pre‑fall assessment and post‑fall care, utilising clinical skills and evidence‑based practice guidelines.
- Analyse, assess, plan, implement and evaluate patients, continuously adapting care plans to empower service users to achieve optimal health.
- Advocate for patients’ best interests, respecting moral values, beliefs, dignity and ethics.
- Analyse and interpret complex situations, mitigating risks and escalating to senior team when required.
- Support the Trust’s redeployment needs to maintain safe patient care and skill mix, working within scope of practice and professional boundaries.
- Hold a professional responsibility in safeguarding vulnerable adults, young people and children, adhering to Trust policies and protocols at all times.
- Collaborate with various multidisciplinary teams to link quality improvement work with procurement and medical equipment, ensuring up‑to‑date equipment and staff training.
- Ability to converse easily and provide advice in accurate spoken English for all customer‑facing roles.
- Most positions require a Disclosure and Barring check; posts working unsupervised with children or vulnerable adults require an enhanced check.
