Our Urgent Care Service delivers responsive, high-quality care 24 hours a day, 365 days a year—supporting people to remain safely in their own homes and communities. From managing acute illness that may otherwise lead to hospital admission, to enabling timely hospital discharge, you’ll play a vital role in transforming patient journeys.
Working as part of a skilled multi-disciplinary team, you’ll provide holistic assessment, planning, implementation and evaluation of care, placing independence and recovery at the heart of everything you do. You’ll help deliver innovative care through emerging models such as virtual wards, supporting patients with acute respiratory conditions using remote monitoring technology.
Night care is equally critical, with dedicated nursing teams providing essential services including end-of-life care, catheter care and urgent, unplanned interventions. In this dynamic role, you’ll manage a caseload of patients with complex needs, delivering safe, effective and person-centred care. You will develop integrated care plans in partnership with patients, ensuring their goals, preferences and independence are prioritised. Clinical excellence is key—undertaking interventions, monitoring for deterioration, and taking prompt, appropriate action.
You will also coordinate care by ordering equipment, making onward referrals and contributing to triage and duty responsibilities. As a valued team member, you’ll collaborate closely with multi-professional colleagues, ensuring seamless care delivery across services.
Supporting the future workforce is equally important. You’ll help guide and supervise junior staff, Health Care Assistants and students, fostering their development and confidence.
