Position Summary We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence while enhancing their quality of life.

Key Responsibilities Provide physical therapy treatments as directed by the supervising Physical Therapist Follow physician orders and individualized treatment plans

Document daily treatments, weekly progress notes, and resident outcomes accurately and timely

Participate in patient care conferences and rehabilitation meetings as needed

Monitor resident progress and communicate updates to the Physical Therapist Assist with cleaning and maintenance of therapy equipment and treatment areas

Train and assist Restorative Aides in carrying out therapy-related programs

Communicate effectively with residents, families, nursing staff, and interdisciplinary team members

Participate in staff education and in-service training programs

Educate residents, families, and caregivers on maintenance programs and discharge recommendations

Report equipment concerns and assist in maintaining a safe therapy environment

Ensure compliance with all facility policies, state regulations, and payer requirements

Qualifications

Education & Experience Graduate of an accredited Physical Therapist Assistant program

Experience in skilled nursing, rehabilitation, or long-term care preferred

Experience with Point Click Care  (PCC) and Casamba preferred

Strong communication, documentation, and organizational skills

Licenses & Certifications Current South Carolina Physical Therapist Assistant (PTA) license in good standing

Must maintain all required continuing education requirements
