Job Summary
The Advanced Practice Provider (APP) works within a clinic as part of a collaborative team, which may include Family Practice and/or Internal Medicine Physicians, RN Case Managers, and clinic staff. This team operates under a comprehensive, coordinated care model to ensure high‑quality patient outcomes.
Key Responsibilities- Initially manage the patient population in collaboration with the primary care physician and/or specialist.
- Provide care in outpatient and/or inpatient settings, with most positions focused on outpatient services unless otherwise specified.
- Deliver both chronic and acute care services to patients, acting as an extension of the primary care physician.
- Use performance metrics tied to the organization’s strategic goals and operational requirements to monitor and evaluate effectiveness and outcomes.
- Perform additional duties as assigned to support patient care and organizational goals.
- Diagnoses and manages both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic illnesses, as well as minor injuries and acute medical issues such as infections or exacerbations of chronic diseases.
- Orders and interprets diagnostic tests, including laboratory work, x‑rays, and EKGs.
- Prescribes medications and other treatments as necessary to address identified conditions.
- Educates patients about their medical conditions, treatment plans, and expected outcomes to ensure they are well‑informed and engaged in their care.
- Collaborates as part of a comprehensive, multidisciplinary team to provide high‑quality, evidence‑based care.
- Graduate credit for PA/NP Program.
- Current Licensure through the Georgia Board.
- Current prescriptive authority from the Georgia Board.
- Ability to lift up to 50 pounds.
- Ability to push or pull heavy objects using up to 50 pounds of force.
- Ability to sit for extended periods of time.
- Ability to stand for extended periods of time.
- Ability to use fine motor skills to operate office equipment and/or machinery.
- Ability to receive and comprehend instructions verbally and/or in writing.
- Ability to use logical reasoning for simple and complex problem solving.
- Ability to travel to multiple locations to support business needs as required.
