Accord Care is seeking a bilingual Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. This role offers the opportunity to provide one-on-one support that promotes client health, independence, and dignity. • Provide skilled nursing care tailored to each client's unique needs
• Monitor health conditions and document changes in clinical status
• Administer medications and perform wound care
• Educate and guide clients and families to support ongoing health at home
• Communicate effectively with the care team to ensure continuity of care

• Licensed as an LPN and in good standing with the Georgia Board of Nursing
• Fluent in both English and Spanish
• Experienced in a clinical setting, with at least one year preferred
• Certified in CPR
• Skilled in EMR documentation, with strong organizational ability
• Confident working independently in a home environment
• Compassionate, client-focused, and a strong communicator
