Accord Care is seeking a bilingual Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. This role offers the opportunity to provide one-on-one support that promotes client health, independence, and dignity. What You Will Do- Provide skilled nursing care tailored to each client's unique needs
- Monitor health conditions and document changes in clinical status
- Administer medications and perform wound care
- Educate and guide clients and families to support ongoing health at home
- Communicate effectively with the care team to ensure continuity of care
- Licensed as an LPN and in good standing with the Georgia Board of Nursing
- Fluent in both English and Spanish
- Experienced in a clinical setting, with at least one year preferred
- Certified in CPR
- Skilled in EMR documentation, with strong organizational ability
- Confident working independently in a home environment
- Compassionate, client-focused, and a strong communicator
