About Facility:

• Outpatient rehab is located 1.5 miles away from the Aurora Bay Area Hospital (55 Bed Community Hospital, 1 hour north of Green Bay)

• Beds: N/A
• Will you accept a traveler with 1-year experience:

• Will you accept a first-time traveler:
  Yes

• Adolescents (approx. 5% of cases and experience not needed) to Geriatrics - general orthopedics, low back pain, hip and knee pain, post-surgical, falls, work-based injuries, general neuro
• Daily Case Load:
  • About 10 patients per 8h Days

• BLS, WI state license
• Bachelor s degree or above

• Manual therapy, exercise, mechanical traction, lite gait, education, PNS (pain neuroscience),
• Preferred: vestibular trained, dry needling, blood flow restrictions

• Other PTs
• 2-3 PTA
• Manager/Supervisor, onsite and via phone
• Schedulers to schedule visits;
• Multidiscipline team with OT and SLP

• EPIC – preferred
• Equipment: hand weights, Thera Band, resistance machines, aerobic equipment, bikes, new steps, treadmills,

• If have a MI license, could float to Michigan Outpatient PT.
• Would know in advance, about 30 minutes away.

• 1-3 days based on comfort

• Weekend rotation:
  None
• Will they be on call:
  No call
• Holiday Expectations: 50% of holidays in contract or every other holidays
  • Clinic is closed day of holidays, but open eves and day after holidays
• Shift times:
  Manager can be flexible on 5 8h shifts or 4 10h shifts. If working 8h shifts, will need to stay until 1730 a few days a week
• Approval of time off:
  Can approve up to 7 days
